PASSPORT Medicaid Waiver

The PASSPORT Medicaid waiver program is for Ohio residents to help Medicaid-eligible older Ohioans get the long-term services and support needed to stay in their homes or other community settings, rather than enter nursing homes.

PASSPORT Eligibility:

• 60 years and older;
• Financially eligible to receive Medicaid institutional care;
• Requires nursing home level of care; and 
• Able to remain safely at home with physician approval
